Yes, Kinship House accepts many insurances including Oregon Health Plan (OHP) and private payment. Child Welfare Assessments are typically funded through the Department of Human Services.

Services are best informed by comprehensive historical client information, so the following types of documentation may be requested at the time of referral. You are also welcome to include these documents at the time of referral without request. Documents can be submitted via email, fax, or delivered in person:
Release of information for the foster parent, therapist, daycare providers, CASA and/or educator
Initial Assessment
Child Welfare Case Plan
CARES Evaluation
IEP Report/Evaluation
Psychological Evaluation
Early Intervention Report

Whoever currently has custody of the child should complete the referral. If the child is in the custody of DHS, a representative from DHS is required to complete. If you have Guardianship or Power of Attorney, please provide documentation at the time of assignment.
