Getting Started

Thank you for your interest in Kinship House services. We're so glad you're here.

A few quick tips

Before you start the referral, these will make the process smoother and easier.

  1. Complete the referral forms on a computer rather than a phone.
  2. Use Google Chrome if possible. Other browsers will work, but Chrome is recommended.
  3. The person with legal custody of the child must complete the referral. If the child is in foster care, the caseworker should complete it.

Intake FAQ

Quick answers to common questions.

Who needs to complete a referral?

Whoever currently has legal custody of the child should complete the referral. If the child is in foster care or in the custody of DHS, the caseworker should complete it. If you have Guardianship or Power of Attorney, please provide documentation at the time of assignment.

Whose information goes on the referral?

The child is the client, so their information is used throughout the form:

  • Address: the physical address where the child currently lives
  • Phone number: the child's current placement
  • Email: the custodial party's email
Who signs the Releases of Information (ROIs)?

The person with legal custody must sign. If the child is in foster care, the caseworker needs to sign these.

What if I'm referring multiple children or siblings?

A separate referral must be submitted for each child.

For caseworkers submitting multiple referrals: our system does not allow the same email to be used as a username more than once. Since each child needs a separate referral, we recommend using the client's first and last name as the username for each submission instead of an email address.

Example: if you're referring a client named Alex Brown, you might use alexbrown as the username.

Please write down the username and password you choose.

What other information do I need to submit?

Services are best informed by comprehensive historical client information, so the following documents may be requested at the time of referral. You are also welcome to include them without request. Documents can be submitted via email, fax, or in person:

  • Release of information for the foster parent, therapist, daycare providers, CASA and/or educator
  • Initial Assessment
  • Child Welfare Case Plan
  • CARES Evaluation
  • IEP Report/Evaluation
  • Psychological Evaluation
  • Early Intervention Report
Do you take insurance?

Yes, Kinship House accepts many insurances including Oregon Health Plan (OHP) and private payment. Child Welfare Assessments are typically funded through the Department of Human Services.
