Our Intake process
How To Get Started
You can contact Kinship House directly and speak with the intake coordinator or you can fill out a referral form on our website and either fax, mail or e-mail it to Kinship House. Contact information is provided on the form.
The Kinship House treatment team meets weekly to review new referrals and assign them in order of receipt and also based on level of urgency. Once we receive your form we will review it and assign it. Please wait for a call from your assigned therapist who should notify you within two weeks to schedule your initial assessment appointment. If you have not heard any information within this time frame or services become more of an urgent need then please contact the Kinship House intake coordinator by phone or e-mail.
If you are requesting an assessment or transitional services that require either Administrative Medical dollars or System Of Care (SOC) the process of assigning a case is different. In the case of Administrative Medical dollars we will need proof of payment or receipt of the actual vouchers before the assessment will be assigned to a clinician and an appointment scheduled. For SOC we will need to review the contract and receive a notice to proceed with services.
